Maybe it would be better to speak about the main heading of the content document without mentioning h1. According to the current guidelines, the main heading of a content document can also be h2. We may also need an explicit rule for how headings like this should be handled: <h1><span class="chnum">Chapter 1</span><br/>Chapter heading</h1> (see section Headings). At least a word space, possible a period or a comma needs to be added after the chapter number.
