Reformat Aldermore Bank CSV for FreeAgent

This is a static page which is intended to be used as follows:

1. Download a CSV from your Aldermore Bank account.
2. View this static page.
3. Use the file chooser to select the CSV from your local machine.
4. Click the "Reformat" button.
5. Click the "Download" button.
6. A CSV file suitable for uploading to FreeAgent banking is downloaded.

Note that the reformatting of the CSV happens in the browser; no data is sent to a server.
