- rewrite appender to support multiple scope + one MDC provider.
- split and rename modules, more opt-in via dependencies
- add examples
- module: web + grpc client combined example (where MDC from gRPC world works)
- without test artifacts (i.e. running closer to real deployment)
- improve docs somewhat
