Renamed since this is replaced with a new version we'll be using to allow end users to report issues on for the Dashboard project (FE, BE, & UI/UX)
New bug report template. Note that the format is fixed since our GitHub Webhook - minervabot - parses new tickets before cloning them to Clickup as Tasks

This change renames the old bug report template and replaces it with a new version that will be used by end users to report Dashboard issues for FE, BE, & UI/UX.
Note that the Minervabot GH Webhook clones new issues in this repo to Clickup as Tasks. Minervabot parses GH issues before creating Clickup Tasks so this issue template should not be randomly changed.
