Tacrolimus (also known as FK-506 or Prograf) is a macrolide isolated from the culture broth of a strain of Streptomyces tsukubaensis. It has strong immunosuppressive properties and is commonly used to prevent allograft rejection in transplant recipients. One of its modes of action is to prevent T-lymphocyte activation. 1 Tacrolimus decreases the activity of your immune system. This may increase the risk that you will get a serious infection. 2

In a study investigating the relationship between low tacrolimus concentrations and increased risk of early acute rejection in adult renal transplantation, one group of patients has tacrolimus levels of 0 - 5 ng/mL 3. Thus, 0 seems like an appropriate lower bound. While normal levels are far lower, one study on toxic levels of tacrolimus included a patient with 50.5 ng/mL 4. Thus, a value of 51 seems like a biologically plausible upper bound.
