Inactive stores were already being excluded from the provider summary's, but it looks like I had accidentally left them in the store listing. Addressing this now to prevent the Boulder Table Mesa King Soopers from showing up for now due to the very sad news there today. It's showing up periodically due to open appointments being detected on the King Soopers site, but I think it's best to disable this for now to prevent any of the automated updates from making it pop up.
