Rather than have the 'add response' button bring up a prompt with two fields (name and response) the two fields should be static-ly displayed on the page and the add response button should enter it.

Why does that feel better than the modal? You could be right, but my intuition doesn't at the moment find an advantage to always-on response fields.

It focuses on ease of entry.
It could be that before your account is associated with a response there is a static input field, and after it is it reverts to being a button. That way when you look at a new fire starter for the first time you are drawn to respond right away.
I imagine it to be eye catching, and less oblique (hidden behind a button).
