Append information to configured pathauto patterns

On the pathauto config page at admin/config/search/path/patterns append a description to each of the content type patterns field #description value.

Text to use:

This content type is configured to use Entity Alias which appends an additional pattern to the alias.

The words configured to use Entity Alias should link to the Entity Alias configuration page.

The #description value appears below the field in Drupal. This is how they currently look without the description.

I assume the above screenshot was taken from Drupal 7, or 6? In Drupal 8 the patters settings view looks like this:

With the updated version of Pathauto, we should append this message to the right of the content type.
