Show debug with hotswap in toolbar by default #177

When I install JRebel, it adds its launchers to the toolbar for easy access.

Even after installing the Vaadin plugin, it can be hard to realize there's a debug with hotswap option because it's hidden behind the dropdown.

Suggestion: add the launcher to the toolbar so it's easy to find.
