Add labels to form fields

AMP flags lack of labels as a severity HIGH problem

Need to add labels such as:

Collection Title

Thanks for testing this Mark. I did log in with Shibboleth, but I am still getting a messaging saying that I "do not have permission to view that report." Can you set permissions on reports, or no? I've not used the AMP tool before.

Nah, no glitch in the shibboleth here - seems like it doesn't take UC Berkeley credentials. When you invited me to view the report, I got a generated password emailed to me.
