Its due to not firing notifications for approved / denied stream access requests.

LOL

Previous report:

This needs further testing, but i've experienced multiple unsent notifications (access requests, mentioned in comment etc) on latest today. It is confirmed by the absence of the related log messages:

These should be there for every notification, but they are missing quite often:
