The first section of the form must collect meta information for relates to List

Shop: Select a shop from the dropdown
Date: default date at the time of the creation
Name: name for the list ( This can be populated from the date + selected shop name)

User should be able to save the selected list

Date, Name, and Shop should be validated for null/empty
