[FEATURE] Add description field for form template

• Update prisma schema, form template dtos and entities such that there is a description field
• You can refer to this PR where I added signatureLink to signature: 213 feature UI changes for logon onboarding and create/upload signature #219
• You don't have to mess with web/src/client, you should be able to run yarn gen-client when the server is running once you make backend changes to auto generate those files
• Make frontend changes to create form template once I merge in [FEATURE] Create form template review and finish pages #206
