I've made a comment about this already in this discussion, but I'm hoping that by submitting this as a bug report discussion there will be a faster resolution to what I'm pretty sure is a bug on GitHub's end.
I've been able to replicate this behavior a few times. When I'm following organizations, I don't see a single "USER starred REPOSITORY" item except for "USER starred YOUR REPOSITORY". When I unfollow all organizations I was following and refresh the homepage, I immediately start seeing all the star events that I was missing. I then re-follow the organizations I unfollowed, wait a bit (guess there's some caching), and boom, no more star events in my feed.
I think this bug is also affecting "USER followed USER" items, but I haven't paid as much attention to that.
I don't have any insight into the code, but I'm guessing that on GitHub's end an error is being thrown because organizations can't have stars, and this error is handled by defaulting to an empty list, resulting in a silent failure.
Secondarily, I think that any time feed items fail to be displayed due to some error, the user should be notified in some way so that they know that some items in their feed may be missing.

I submitted a support case. A member of support was able to recreate the issue, and it has been flagged for an engineering team to investigate.
