Add issue templates

This is a placeholder until we post more details. The intent is to put appropriate issue templates so that issues opened in this repo are similar to anax and other repos. That will either involve creating issue templates under /.github in this repo, or even better, identifying templates that apply to all repos and posting those in the .github repo.
