Fatal error detected - Failed to execute script POE_QOL2

Trying to use the app for the first time but I'm constantly running into this error. I've tried all the different versions and gone through all the similar issue threads but none of the solutions work. I am sure I am setting up the Setup.ini file correctly and the poeqol2_logfile is returning -

Really wanna try this app out for Ultimatum, any ideas on how I can get it working?
