All content titles should come from metadata.json, not from path #65

In a few cases, titles are pulled from path information and not from metadata.json.

I can update this:

Titles come from the metadata.json. However, the Table of Contents and breadcrumbs are generated from file structure.

It would be a nice feature to have article titles instead of article base paths, but this isn't really too important.
