The heart age tool asks this. The questions in the tool are based on the JBS3 Cardiovascular Risk Assessment.

In the GP registration service we ask people if they have ever smoked, then for people who say yes, we ask them if they smoke now or used to smoke. Finally, we are looking to add a question that asks them how many they smoke in a day.

For the 'How many' question, we have gone with radio options that match SNOMED codes that GPs enter into their system
