Configurable form for *reviewers*

Something that we've been discussing for a while that would be a great help is for reviewers to be able to do more than just the basic approve/reject/request changes/comment.

We should allow an additional "form" page/section which is just for reviewers, where they can select from drop-downs, enter other data, and generally have their own "application form" which can be considered in the actions and affect outcomes etc.

This form would appear at the bottom of their review page. The "default" would be what they have currently -- Option to select the outcome, and an overall comment.

Decent amount of work to implement, but I'd put this fairly high on the list of "things we'd like to have".
