An inactive character lights up a cigarette after a short time of standing in place. When she finishes the cigarette (cca 10 seconds), she experiences a gruesome death, leaving (disconnecting from) the game.
This could complement the party "drop in - drop out" play.

Possible extension: The character gains a small temporary advantage (faster movement, dexterity etc.) after smoking for a while (if the player picks up the controller before the character dies).
I've always wanted to make a platformer with a protagonist that smokes when he's idle. It's also a reference to old platformers, for example Commander Keen (whose protagonist reads a book when idle) or Comix Zone (whose protagonist smells his shoe when idle).
