Not at all urgent:

Since the FSF address has changed, as noted in #100, it may be a good idea to update the license boiler plate in source files:

  • replace the old physical address with http://www.gnu.org/licenses/
  • add Copyright (C) 2007 Lawrence Livermore National Security, LLC to reflect LLNL management change
  • address inconsistencies like vestigual boxes for CVS/svn id tags
  • drop author from boiler plate

I didn't check to see if we have other copyright claims in the code, but obviously we'd need to preserve those.
