Does our fhir facacde support FHIR R4?

Seeing some updates related to FHIR R4 in this version https://github.com/Asymmetrik/node-fhir-server-core/releases/tag/1.4.0 .

How can we use FHIR R4 standard in fhir-facade-starter?

We do have R4 support in 1.4.0 and 2.0.0. I'm going to update the starter to show examples on how to use it.

1.4.1 and 2.0.0 will be published today.
