Add theme note to the docs

Add a note to avoid confusion. As a theme can mean two things.

At the top of the README when you say this is a Jekyll theme, you should add that one cannot add this as a configured theme on an existing Jekyll site, but that one should create a copy of the repo and then use the content as a theme.
