It seems that the gem does not support accessing to Okta behind a http proxy by default.

I came across this stackoverflow post and was able to make it work.

However, it would be nice if we can do something like the omniauth-twitter gem to support proxy and use env variables
like they did in this pull request

@blethier Thanks for leaving an issue. I can look into it. Just curious what is the usecase? Why use a proxy versus just hitting Okta directly?
