Describe the bug
Prophet does not autostart with VSCode. It does not appear in the Switch Output menu. The debugger would not start on F5.
It only starts after I click on the Prophet button on the Primary Side Bar on the left.

To Reproduce
Steps to reproduce the behavior:

  1. Launch VSCode
  2. Check the Switch Output menu
  3. Observe that the Prophet Uploader is not present there.

Expected behavior
Prophet plugin starts automatically when VSCode starts. Uploader is working according the current settings. Debugger will launch when F5 is pressed.

Desktop (please complete the following information):

  • OS: Windows 10
  • VSCode version: 1.79.2 (user setup)
  • Node.js: 16.17.1
  • Prophet v1.4.33 is the last version without this bug. Starting from 1.4.34 and to the 1.4.40 this bug is present.

Additional context
It looks like this bug is intermittent. I know some people faced it too. Some of them solved it by reinstalling the plugin or VSCode. I, however, installed VSCode and the plugin on a new laptop from scratch and the bug haven't gone away.
