Conflicting info and ambiguity in existing data

As per Eluminator's post:

Some mechanics in PoE are not clear or there is conflicting information about it.

For example: the current page about paradoxica says that it "drops from any Betrayal bosses in the Safehouse area".
While some community members say it only drops from interrogating the intervention leader others say they got it from somewhere else. Somebody reading the current wiki is not aware of that dilemma.

Suggestion: If something is stated in the wiki there should be a source for it.
I know that is a lot of work, but having links to direct quotes form GGG employes would be ideal.
If something has not been tested and is just a theory it should also be stated. I know I ask for a lot, but I am also willing to help with that (as that is a manual process :/ )
