Restored Ultra-Violence+ difficulty

WadSmoosh used to have an "Ultra-Violence+" difficulty, like the one from the Unity ports of Doom. I deemed it a redundant feature, so it was removed from WadFusion. For those who really want to have it anyway, here is an addon for it.

You can rename the zip archive to pk3, but it's not a requirement. GitHub just doesn't want to attach files with unrecognised extensions.

To summerise what it does, UV+ is the same as UV, but with the effects of the -fast parameter, and with multiplayer things in singleplayer.
