Add updatable Comment field to the Rename Structure Field dialog.

Is your feature request related to a problem? Please describe.
When discovering structures, sometimes adding a comment (optionally with the listing's address) as to where/why the name changed helps if you're later found to be wrong and need to retrace.

Describe the solution you'd like

1. Add the Comment field to this dialog prefilled with any existing text.
2. Optionally add configurable tick boxes to prepend the address/location and maybe the current date/time to the comment text.

Describe alternatives you've considered
Finding said datatype, opening it, scroll to field, modify name and comment fields
