Create an Issue Template for all of my repos/projects.

intended to be an issue that is used across my 4 Projects.

This issue applies to all for.

Once I complete this task, it will be used for each of those projects/repos

Specifically the line that says If a related issue doesn't exist, you can open a new issue using a relevant [issue form](https://github.com/github/docs/issues/new/choose)..
