<p>Pt presents for prep, temp and final impressions for tooth # porcelain crown. Med hx reviewed, no changes. The existing crown was removed. The prep was refined. An acrylic temporary was made. Pt was anesthetized using topical benzocaine followed by <1 carpule of 4% septocaine w/ 1:100k epi via infiltration. "000" and "1" retraction cords soaked in hemodent were inserted into the sulcus. The "1" retraction cord was removed then the final impression was taken. The "000" retraction cord was removed. The temporary was cemented with TempBond. All excess cement was removed. Occlusion was checked. Pt is comfortable with her bite. POI given to pt. Shade B1 was chosen.</p>
